RESUMO

The dynamic hip screw (DHS) is a widely used method of osteosynthesis. Nevertheless, supplying instable pertrochanteric fractures is problematic because the gliding screw may cause a considerable dislocation of fragments in lateral direction. Is there any alternative method disposable, bearing stability of DHS- osteosynthesis can be improved by adding a lateral T-plate. We reached full bearing stability in four cases.

RESUMO

The treatment of coxarthrosis with the cemented Duopleet prosthesis represents an efficient alternative to total hip prosthesis in elderly and polymorbid patients. The early results show a tolerable rate of complications. The patients can be mobilized early. The mortality seems to be low. Medium-term results are satisfactory. The rate of protrusions is lower than in patients with common femoral head prosthesis. Paraarticular ossifications are seen frequently. In numerous patients we found pain with projection to the operated hip.

RESUMO

The chances for conservative treatment of bleeding ulcers have been expanded by progress in endoscopic haemostasis. Surgery, nowadays, is in many cases applied only to patients with uncontrollable recurrent haemorrhage, after the internist's efforts have failed. This has increased lethality from 20 to 44 percent. To improve results, elective operation is recommended in the no-bleeding interval, following endoscopic haemostasis.

RESUMO

A 68-year-old man developed severe swallowing difficulties after a cerebrovascular accident: he was unable to take food. Nutrition was assured via a gastrostomy ( Witzel fistula). Nonetheless there was steady weight loss. A cricopharyngeal achalasia was finally discovered to be the cause of the swallowing problem. Myotomy of the cricopharyngeal muscle completely abolished it.

RESUMO

The prognosis of carcinoid tumors of the Gastro-Intestinal Tract (GIT) is determined by the site and size of the tumors. Carcinoid of the vermiform appendix and the rectum cause early symptoms making early diagnosis and treatment relatively easy. The 5-year-survival rate is 99% and 83% respectively. Carcinoid tumors involving the stomach and the colon with 5-year-survival rates of 52% and 53% respectively have an unfavourable prognosis. In between is the carcinoid tumor of the small intestine with a survival rate of 60%. Tumors with diameter less than 2 cm can be excised locally. Bigger tumors--as a rule--must be managed according to the principles of tumor surgery. The operative treatment of metastasis improves the prognosis especially of small intestine carcinoids. One should endeavor to remove also metastasis of carcinoid tumors of other localizations.

RESUMO

Of 51 patients with primary hyperparathyroidism (2 patients with MEN, Type 1 clinical symptomatology, diagnostic procedures, differential diagnosis, operative strategy and long-term results are being reported. Aside from clinical findings and radiologic signs in our hands determination of the ionized serum calcium fraction, results of chrest bone biopsies and parathormone determinations are best parameters to substantiate the diagnosis of PHPT. Parathormone radioimmunassay determination is very helpful in localizing the adenoma, especially in cases of reoperations. Five patients were seen in acute hypercalcemic crises, in which emergency operations are absolutely indicated. Postoperative hypercalcemia and recurrencies were observed in 3.9%. Successful extirpation of parathyroid adenomas (15% multiple adenomas were found) is the therapy of choice in PHPT, only in cases with hyperplasia subtotal parathyroidectomy is indicated.

RESUMO

Osteons of Beagle-ulnae were calculated in growth. Controlled osteons were compared with osteons after "therapy". Stimulation was made with electro-nails system Kraus-Lechner. The mean-values of osteons after "therapy" surpass control. The fundamental question, whether induction of bone growth occurs with low frequency electromagnetism will in all probability be have to answered in the affirmative.

Chondrogenic tumors of the skeleton are divided into 6 separate groups: 1. chondroblastomas 2. chondromyxoidfibromas, 3. osteochondromas (cartilaginous exostoses), 4. enchondromas, 5. periostal chondromas, and 6. chondrosarcomas. The presentation of the different types of tumors demonstrates the pathogenesis, pathology, clinical findings, treatment, and prognosis. States of tumors before and after operative treatment are illustrated.
